Terms & Conditions + Parent/Guardian Consent

Welcome to the POISED to Prosper Online Program, a Quartz Wellness Collective initiative designed to support the personal, social, emotional, academic, and career development of young women. Because participants are under 18, enrollment requires parent/guardian consent and agreement to the following Terms & Conditions.


Please read this document carefully before enrolling your child.
 
1. Program Overview
POISED to Prosper is a live, interactive, online program for participants ages 13–18. Through weekly virtual sessions, activities, coaching, and digital content, participants engage in learning experiences focused on leadership, self-awareness, career readiness, and personal development.
 
2. Eligibility
To participate in the program, participants must:

  • Be between 13 and 18 years old.

  • Have access to a device with video and audio capability.

  • Have a stable internet connection.

  • Have a parent or legal guardian who completes this consent form.
     

3. Parent/Guardian Consent (Required)
By enrolling your child in this program, you confirm that:

  • You are the parent or legal guardian of the child being registered.

  • You consent to your child’s participation in all POISED to Prosper online sessions, activities, coursework, and community spaces.

  • You understand that sessions may involve the use of video conferencing, digital collaboration tools, generative AI, and communication with other youth participants.


4. Use of Video, Audio & Online Engagement
Because POISED to Prosper emphasizes connection and community:

  • Participants are expected to appear on camera during live sessions.

  • Sessions may be recorded for quality, training, and internal program development.

  • Participants will engage with facilitators and interact with other students in breakout rooms, chat, and group activities.
     

5. Media Release & Content Usage
A. General Permission
By enrolling, you grant Quartz Wellness Collective permission to:

  • Capture photos, screenshots, or short video clips of your child during sessions.

  • Use these images for promotional, marketing, and educational purposes, including but not limited to social media, websites, newsletters, brochures, and program documentation.
     

B. Use of Voice & Full Video Footage

  • If we wish to use extended video clips, audio recordings, or identifiable spoken content featuring your child, we will contact you for written approval before publishing.

  • You may decline permission for promotional use at any time, though this may limit certain program activities.
     

If you prefer to decline media permission, please indicate this on the enrollment form; your child may still participate fully. If you have any questions regarding this matter, email hello@39quartz.com.
 
6. Artificial Intelligence (AI) Tools Usage
POISED to Prosper uses AI tools (including ChatGPT) to support learning activities such as:

  • Brainstorming

  • Writing prompts

  • Skill-building exercises

  • Research

  • Interactive challenges
     

We commit to ensuring:

  • AI usage is age-appropriate and educational.

  • Personal identifying information is not required for AI interactions.

  • Activities comply with COPPA (Children’s Online Privacy Protection Act) guidelines.
     

By consenting, you agree to allow your child to use AI within the structure and supervision of the program.

7. Online Safety & Conduct Expectations
Your child's safety is our top priority. All participants must adhere to the following expectations:

  • Treat facilitators and peers with respect.

  • Refrain from bullying, harassment, discrimination, or inappropriate behavior.

  • Keep all personal information private (e.g., addresses, phone numbers, social media accounts).

  • Use program platforms only for program-related activities.

  • Follow facilitators’ instructions and community agreements.
     

Quartz Wellness Collective maintains a zero-tolerance policy for harmful behavior. Participants may be removed from the program without refund for violating safety or conduct guidelines.

8. Privacy & Data Protection
Quartz Wellness Collective:

  • Collects only the information necessary for enrollment and participation.

  • Does not sell or share personal data with third parties.

  • Stores data securely and in compliance with COPPA, FERPA (where applicable), and general data privacy standards.

  • Limits access to recordings and participant information to staff and contracted facilitators.

 
9. Technology Requirements & Responsibilities
Parents/guardians are responsible for ensuring:

  • Your child has access to a functioning device, Wi-Fi, and a private or semi-private space.

  • You supervise your child’s online participation in a manner appropriate for their age.
     

Quartz Wellness Collective is not liable for:

  • Internet outages or technical issues

  • Loss of access due to device failure

  • Missed sessions due to technology limitations


10. Program Materials & Intellectual Property
All curriculum materials, worksheets, videos, activities, and session recordings are the intellectual property of Quartz Wellness Collective.
Parents and participants agree not to:

  • Share program materials publicly

  • Reproduce or distribute content without permission

  • Record or capture screenshots of sessions


11. Health, Well-Being & Emotional Safety
POISED to Prosper is not therapy. Although facilitators are trained in restorative practices and positive psychology:

  • The program does not provide mental health treatment or crisis intervention.

  • You agree to inform us of any allergies, medical conditions, learning needs, or accommodations your child may require.

  • You acknowledge that you remain responsible for your child’s well-being and supervision throughout the program.

 
12. Cancellations, Withdrawals & Refunds
Because seats are limited and facilitators are scheduled based on enrollment:

  • Program fees are non-refundable once the program begins.

  • If you withdraw before the program start date, refunds may be granted at the discretion of Quartz Wellness Collective.

  • If your child is removed for conduct or safety violations, no refund will be issued.

13. Liability Release
By enrolling your child, you:

  • Release Quartz Wellness Collective, its employees, and contractors from liability for injuries, damages, or losses arising from program participation, except in cases of gross negligence.

  • Accept responsibility for your child’s internet security, physical environment, and supervision during online sessions.

14. Parent/Guardian Acknowledgment & Agreement

By checking the box and completing enrollment, you affirm that:

✔ You have read and agree to the POISED to Prosper Terms & Conditions.
✔ You consent to your child’s participation in the program and use of digital tools including AI.
✔ You give permission for your child to appear on camera and be recorded during sessions.
✔ You grant permission for Quartz Wellness Collective to use photos/screenshots for promotional purposes, with extended video/audio usage requiring additional approval.
✔ You understand the expectations, risks, and responsibilities associated with online learning.
